Position Description

Meals on Wheels People is looking for an In-Home Client Coordinator to join their team and conduct eligibility assessments for home delivered meal clients residing in Multnomah, Washington, and Clark Counties. This full-time position requires an experienced candidate who will act as a liaison between contract partners, participants, and staff to ensure accurate authorization and delivery of services. The ideal candidate will exhibit proficiency in computer applications, handle escalated client concerns and comments professionally, and have good recordkeeping and organizational skills.

Responsibilities

– Serve as a liaison between referring partners, contract partners, clients, and staff
– Provide direct one-on-one and phone screening with clients
– Notify appropriate parties of new starts and changes promptly
– Communicate with referring partners, case managers, clients, and staff regarding the status of authorizations and potential disruptions of services
– Handle escalated client concerns and comments in a constructive, professional, and helpful manner
– Ensure integrity of data in client data management system, update client information as needed, and correct inaccurate information as necessary
– Conduct home delivered meal eligibility assessments annually in accordance with the Meals on Wheels People Operating Procedures
– Assist in reducing barriers to service by building and increasing communication with community partners and case managers and increasing awareness of community resources across the organization
– Train staff members on principal duties within Client Services as needed
– Communicate with center staff to ensure efficient delivery of home-delivered meals
– Coordinate volunteer projects and voucher processing as assigned
– Attend all staff meetings and trainings
– Other duties as assigned

Qualifications

– Associate Degree or equivalent job experience in Gerontology, Counseling, Human Services, or Communication
– 3 years’ experience in direct client services on social service environment preferred
– Bilingual/Bicultural: English/Spanish, Vietnamese, Russian, Asian or African languages preferred
– Good recordkeeping and organizational skills
– Ability to manage time effectively
– Ability to be flexible as new tasks arise
– Ability to quickly learn new computer programs; experience with client management systems a plus
– Ability to communicate with diverse populations

The successful candidate will have to work remotely and will require a COVID19 vaccination or approved waiver. The job demands include sitting for extended periods of time, ability to climb stairs to complete client visits, and frequent travel throughout Multnomah, Washington, and Clark Counties required.
