Principal obligations:
- Talks with members one-on-one and does telephone screening and assessment to determine the most approximate level of care that will help members overcome obstacles and address socioeconomic determinants of care gaps.
- Examine the clinical requirements for the various programs offered by Optum, our client, and their external vendors.
- Choosing the best program to offer and getting permission for programs like CM, Chronic, MSK, BH, Palliative, and Maternity
- Also involves helping the member coordinate their care (for members who do not satisfy program requirements or decline); helps them with things like finding the right services, using the client wellness portal, etc.

Required credentials:
- RN license that is valid and unrestricted in California
- two or more years of practical nursing experience
- Experience using a dialer system for outbound calls with contact center nurses, telephone triage, and/or case management for 1+ years
- Must be prepared to rotate weekends and nighttime shifts as follows:
- 10:30 a.m. to 7 p.m. (PST) or 11 a.m. to 7:30 p.m., and every third month on average.
Preferred credentials:
- Certification in Case Management and a license for CCM Compact Nursing
- Excellent telephone skills with the capacity to evaluate, discuss, and offer solutions based on member need. Exhibit excellent nursing process skills and the ability to prioritize symptoms presented by the member.

- Excellent communication skills in writing. The documentation reflects a clear picture of the member’s condition and requirements and is accurate, timely, and pertinent.
