  • Identifies and removes social determinants of health and other obstacles that prevent patients from following their treatment plan.
  • Assesses, plans, coordinates, monitors, and evaluates the care plan to make certain that each person receives the services and assistance needed to satisfy their psychological and medical requirements.
  • Coordinates care, has dialogues about goals of care, and involves the patient and family in advance care planning.
  • Offers individuals and families who are coping with chronic disease and/or changes in functional status brief psychosocial help.
  • Attends clinical rounds to work with the clinical care team to meet the patient outcomes that have been set.

Minimum Requirements

  • Current Texas state license as an LCSW
  • Expectations include sitting for or submitting an application for cross-licensure in other states 90 to 120 days after hire. (Licensing fees should be paid at time of employment.)
  • The last three (3) to five (5) years in clinical practice

(Experience) Preferred Qualifications

  • Clinical experience in one of the social work specialties listed below: Coordination of care at the hospital, community, and population levels
  • Long-term care, rehabilitation, and specialized nursing
  • Social Work in Managed Care and Geriatrics
  • Palliative care, hospice, and home health

Occupational Skills:

  • Outstanding organizational, time management, research, analytical, negotiating, oral and written communication, and interpersonal skills
  • Strong abilities in the following areas: cooperation, dispute resolution, assessing difficult challenges, ability to make adjustments, and making wise decisions
  • Understanding of the guidelines and regulations for managed care and utilization reviews
  • Demonstrated cultural awareness toward situations, participants, and clients.
  • The capacity to adapt to changing business environments and strike a balance between patient advocacy and commercial needs
  • Ability to mentor and instruct patients and other staff members
  • Strong computer literacy, familiarity with electronic medical records, and knowledge of Microsoft Word, Excel, and Outlook are all desired.
