Responsibilities:
- To perform initial health evaluations for newly recruited patients in our network, we are accepting transfers from the Patient Enrollment team.
- Answer patients’ inquiries and start a candid conversation to learn about their requirements.
- Determine and arrange for patients to have access to community services that will improve their care.
- Educate patients and the public about managing chronic illnesses through health literacy and education.
- Help with medication management, which includes arranging refills, recognizing any drug issues, and maintaining adherence.
- Help make sure that your patients receive home health, DME, home infusion, and other essential treatments on schedule.
- This person needs to be able to adjust to a fast-paced work environment rapidly. The most of your shift in this capacity must be spent on the phone.
Requirements:
Extra prerequisites:
- License (LPN) for Active Compact/Multi-State (LVN)
- Proficiency in technology: Microsoft Office Suite
- Exceptional communication skills both in writing and speaking
Requirements for Remote Work:
- Need to have high-speed WiFi turned on.
- Need to have a HIPAA-compliant workstation or a home office
