The Health Coach uses the member’s benefits to educate and address any barriers to adherence in order to improve medication adherence and/or HEDIS diabetes and blood pressure control among our Medicare population. Outreach to pharmacists and doctors may be part of this. The Health Coach involves, educates, and promotes/influences members in decisions related to reaching and maintaining optimal health status through a collaborative process of assessment, planning, implementation, and coordination.
Responsibilities:
- Members are assessed using motivational interviewing techniques, a variety of tools, and healthcare professionals with experience in health risk behaviors. This assessment is done to determine whether the member needs medication adherence counseling or HEDIS health screening/control.
- Encourages behavior adjustments to achieve the highest degree of health by using influencing and motivational interviewing techniques to ensure member involvement and identify health requirements based on key questions and conversation.
- Identifies potential for integrating into existing internal or external projects in collaboration with members
- Taking a comprehensive approach, seeks advice from the manager or others on how to get around obstacles that prevent helping members reach their goals and objectives.
- To enable the member to make continuous, independent, healthy healthcare decisions, coaching, information, and support are provided through interviewing and active listening techniques.
- Interprets and applies regulations/criteria to improve the health of members
- Gives current medical information to make it easier for a member to comprehend their health state
- Assists the member in making informed healthcare decisions with their provider.
- Use discretion when deciding whether to include methods meant to address complex socioeconomic and health indicators that affect member issues, healthcare access, and service delivery, as well as risk factors and impediments.
- Identifies possibilities to improve a member’s overall wellbeing through integration and develops a proactive plan of action to address issues brought up to improve short- and long-term health results.
Pay Scale
The usual pay scale for this position is as follows:
- At least 43,700
- Up to 100,000
Please remember that this range represents the salary range for every position in the job grade that this position belongs to. The location is just one of several variables that will be considered when determining the actual compensation offer.
Requirements:
Required credentials
- 3+ years of experience working in the medical industry
Recommended Requirements
- Highly preferred: healthcare licensure and/or certificates (RN, LPN, MSW, LCSW, CDE, etc.).
- 5 years of clinical practice in the areas of oncology, primary care, diabetes, and hypertension
- Experience with Managed Care and Medicare
- Prior knowledge of coaching, instruction, or case management over the phone
- Excellent computer abilities, including knowledge with MS Office programs
- Autonomous worker
- Outstanding time manager
- Outstanding intellectual and problem-solving abilities
- Having good interpersonal, organizational, and communication skills is a bonus.
- Ability to properly interact with others, communicate, solve problems, and utilize excellent judgment in trying circumstances
- Expertise in leading clients to modify their behavior for the better
- Active listening abilities and training in motivational interviewing.
