PrescottWider Circle is seeking a Bilingual Community Health Engagement Associate in Prescott/Phoenix, AZ, to join their team. This role is integral to running the Connect for Life program, aimed at connecting neighbors for improved health and well-being. The successful candidate will exude confidence and empathy, build trust with members, and adeptly switch between group and one-on-one interactions. Responsibilities include organizing events, conducting home visits, telephone outreach, and fostering community connections to combat healthcare disparities.

Location: Prescott/Phoenix, AZ

Skills: Community outreach, healthcare knowledge, communication, event planning, networking, organization, computer skills, bilingual (English and Spanish).

Responsibilities:

  • Lead the Connect For Life program using Wider Circle’s curriculum to promote health and trust.
  • Plan, promote, and host virtual and in-person events to encourage member relationships and wellness.
  • Reach out to members, ensuring their participation in Chapter events and group activities.
  • Conduct home visits to review benefits and share health plan resources.
  • Document member interactions and identify potential leaders.
  • Connect with prospective members and enroll them in the program.
  • Act as a liaison, linking members to health plans and local resources.
  • Establish partnerships with community-based organizations.
  • Monitor and report on membership engagement and growth.

Requirements:

  • Passion for helping and serving others.
  • Understanding of socio-economic and public health challenges facing disenfranchised populations.
  • 3+ years of relevant community outreach, teaching, facilitation, or healthcare experience.
  • High school diploma or GED; bachelor’s or associates degree preferred.
  • Strong presentation and communication skills.
  • Outgoing personality, positive attitude, and ability to engage with others.
  • Strong organizational and time-management skills.
  • Proficiency in computer systems and web-based tools.
  • Ability to work independently and travel to members’ homes.
  • Flexible schedule, including evenings and weekends.
  • Commitment to a drug-free workplace.
