Responsibilities:
- Onboarding of new patients
- Use video visits to have first consultations with Firefly patients.
- Make long-term care plans by collaborating with each patient to develop a list of objectives and related activities.
- Management of chronic diseases
- Create care plans for patients with particular diseases (such as diabetes or hypertension) to assist them in controlling and managing their chronic illness.
- Work together with Health Guides, who will visit patients via video on a regular basis to assist them in reaching their objectives.
- Work together with the behavioral health team to identify, diagnose, and treat mental health conditions that belong in a primary care context.
- Utilize a network of virtual professionals and collaborate across disciplines to diagnose and treat more complicated illnesses.
- Taking care of urgent complaints
- Assist RN staff in utilizing the Firefly mobile application or phone calls to assist in the triaging and resolution of new patient complaints.
- Oversee care plans to ensure follow-up and to include new concerns.
- Wellness and preventive
- Together with patients, establish health and preventive objectives utilizing accepted preventive templates (e.g. cancer screenings, vaccines)
- Create and use resources to assist patients in managing their wellness objectives.
Requirements:
- Possess a state-accredited nurse practitioner degree possess board certification ANCC, AANP
- Possess a valid license to work as a nurse practitioner in Massachusetts and in at least one of the states listed below: TN, SC, RI, PA, NC, DE, CT, NY, NJ, MD, WV, VA, VT, TN
- possess a valid Massachusetts Controlled Substance Registration and federal DEA registration.
- able to perform a shift from 11 to 7 EST
- possess expertise providing patient care in teams while functioning well
- possess outstanding verbal and writing communication abilities
- possess a minimum of two years’ worth of primary care experience
- possess telemedicine expertise (a plus)
- Fluency in Spanish is a bonus.
- Possess knowledge of process improvement tools (a plus)
