As a vital member of a small RFIC team, you will work to research, design, and put sophisticated CMOS technology nodes’ high-volume manufacturing of the next generation of wireless technologies.
Care Coordination: Arranges for the targeted, high-risk, and post-transition populations to get care in an appropriate, effective, and affordable way.
Helps the patient set up prompt access to resources and assesses the appropriateness of social, economical, and environmental support in a culturally sensitive way. Care Planning: As needed, collaboratively develop/update care plans for disease management, care transitions, and other topics. communicates with the primary care physician and the healthcare delivery team while assessing the efficacy and relevance of the treatment plan. seeks advice from doctors and other members of the medical staff as necessary to update the treatment plan. prepares for and takes part in Care Team meetings or huddles to discuss complex care issues. Disease Management/Outreach: Establish new and continuing telephone interactions with customers in accordance with policy for a designated caseload. Improve the patient/caregiver interaction. To support and manage the “coordination of care” needs of discharge planning, identify and close care gaps, and provide education within predetermined deadlines, complete post-discharge calls to all identified patients. Use of Internal/External Resources: Identifies and directs suitable patients to relevant Arizona health plans and ENCG programs that are eligible for them. Review and advise the patient/caregiver of the available care options, how to use benefits, and what local resources are available. Assist with the transition of care and hospice resources, direct acute admits, direct skilled nursing admits, home health/infusion needs, and durable medical equipment for patients and practices with high-risk members. act as the patient’s or the caregiver’s point of contact, advocate, and informative source. Use well-known community resources to connect patients with available services and programs. Maintains up-to-date clinical knowledge about the important aspects of the target population, such as disease conditions, quality standards, usage patterns, and clinical treatment recommendations. The following illness states are specifically targeted: COPD, CHF, diabetes mellitus, and depression. To improve quality outcomes with the proper support services in managing pharmacy needs, coordinate and identify high-risk populations with a history of polypharmacy. Documentation: maintains the necessary paperwork and tracking as required by the PHIT Department and ENCG. Other obligations meets the requirement for clinical supervision of the LPN examining care plans and needs assessment of the patient. Participate in a cross-functional project team as a representative of the PHIT department. more tasks as needed.
Requirements:
- Required: Current Arizona RN License
- Preferably, you should have 4-5 years of nursing experience.
- Preferably with nursing experience in cardiac, pulmonary, or diabetes
- Experience in case management, care coordination, or disease management is preferred.
- Preferably with certification in a related sector
- Excellent communication skills both in writing and orally,
- Knowledge of documenting electronic health records
- Experience with workflows, process optimization, and cross-functional team cooperation
- Strong command of Word and Excel on Microsoft platforms
- Must be able to work well in a group setting and demonstrate adaptability and passion for swiftly picking up new knowledge and abilities.

- Working cooperatively with Licensed Practical Nurses as a member of the care delivery team has been demonstrated.
