Working in this position will allow you to be part of a cutting edge virtual diabetic clinic that significantly increases the number of seniors in America who can receive specialized care. In order to conduct these visits, you will also make use of and contribute to the improvement of our in-house technology and electronic health information platform. You will collaborate directly with the doctors and other APRNs in our virtual specialty clinic at Devoted Medical on a daily basis, in addition to medical assistants, clinical guides (nurses, dieticians, and health coaches), and pharmacists. The virtual specialty care programs offered by Devoted include the diabetes clinic. These clinics function as small centers of excellence that provide patients with chronic conditions such as diabetes, hypertension, chronic obstructive pulmonary disease, and congestive heart failure with individualized, highly specialized care.
Responsibilities:
- During virtual consultations, conduct in-depth and targeted assessments of patients with diabetes. This may involve ordering sophisticated tests, analyzing lab and imaging data, and creating a treatment plan in conjunction with the team at the specialty care clinic.
- Provide precise diagnosis and customized treatment programs for diabetic patients, including advice on medication administration, lifestyle changes, and follow-up care.
- In order to coordinate comprehensive care for the member, guarantee continuity of care, and provide a collaborative care plan, work closely with other members of the care team, such as PCPs, endocrinologists, and other Devoted team members like pharmacy, clinical nursing, social work, and certified diabetes educators. You should also communicate with family members and caregivers.
- During business hours, act as the specialty clinic’s clinical adviser and offer clinical escalation support to the staff and other teams.
- Take part in regular panel review sessions to provide your knowledge and counsel regarding medical management.
- Conduct a thorough assessment that covers social and home health/DME needs, medication reviews, thorough diagnosis/disease reviews, and evaluations for quality of care (STARS/HEDIS) interventions.
- Use our in-house developed electronic health information system for appointments and offer suggestions for interface enhancements.
- Ensure that patient records are current and correct while adhering to all applicable legal and ethical requirements.
- To stay up to date on the latest developments in diabetes care and best practices, take part in quality improvement programs and continuing professional development.
- Respect all applicable laws, rules, and industry guidelines, particularly those pertaining to patient privacy and telehealth.
- Make urgent visits to assist our clinical nurses in triaging member calls during on-call shifts. Every quarter, at least one four-hour on-call duty must be completed on a weekend or holiday.
Requirements:
- competent nurse practitioner with extensive knowledge of managing diabetes, including the disease’s course, available treatments, managing medications, and changing one’s lifestyle.

Required qualifications and experience:
- APRN with at least five years of experience in outpatient clinical practice, preferably in a clinic that focuses on diabetes or endocrinology.
- A minimum of two years’ worth of experience, primarily in diabetes teaching and management, is needed.
- Proficiency with electronic health records (EHR) and telehealth technologies.
- It is recommended if you have prior experience providing virtual care and have a strong willingness to carry out virtual visits and clinical nursing practice in the community because you support the goal of providing care where patients live.
- It is advantageous to have knowledge of managed care, including how to categorize clinical comorbidities, detect clinical care gaps, and evaluate STARS/HEDIS metrics appropriately.
- For this role, proficiency in both Spanish and English is necessary.
- It is necessary to be licensed in many states and to be willing to seek and maintain new licenses upon request.
Authenticity and Credentialing:
- A master’s or doctorate in nursing with a focus on diabetes treatment.
