Responsibilities:
A Community Connector is a client-focused position in charge of using technology to help clients with complex needs receive evaluation, problem-solving, and referral support over the phone. A effective Community Connector is aware of the value of empathy, advocacy, cultural sensitivity, and follow-up support in assisting clients in gaining access to the resources they need to construct and maintain healthy lives. A creative intellectual with strong critical thinking abilities and a passion to aid those in need is needed for this position.
Take Initiative and Change the World
- Conducts thorough screening for community programs in order to inform, direct, and link clients with necessary resources.
- Determine programs to help the client based on the social determinants of health using a needs assessment.
- Develops a plan that identifies solutions to access barriers by actively listening to customers.
- Accurately locates and recommends clients to pertinent community services that could take care of their further social and medical requirements.
- Completely records all client interactions and unmet needs in 2-1-1’s systems to help find service gaps that can be filled.
- Aids clients in finding local services and resources.
- Recognizes hurdles to care, offers support, and keeps track of client development.
- Provides high-quality, confidential connections to resources and services for all 2-1-1 customers by utilizing active listening and conversational intent in all contacts.
- Monitors and continually enhances personal productivity, quality evaluations, client satisfaction surveys, and efficiency to ensure successful service delivery.
- Conducts follow-up conversations with clients to find out whether they require advocacy, additional services, or feedback on the services they have received.

Qualifications
- High School Graduation
- 1-2 years of hands-on client-facing experience working for a non-profit, neighborhood-based health or social services agency, information and referral service, counseling facility, or philanthropic organization.
- Knowledge of or training in the humanities and social sciences, such as public health, social work, psychology, sociology, and/or human development.
- The capacity for critical thought, creative problem-solving, and successful client assistance in trying circumstances.
- Positivity, a willingness to learn and develop, and leadership ambitions.
- An emphasis on the development and welfare of individuals as well as the communities to which they belong.
- Outstanding interpersonal, written, and vocal communication abilities.
- High degree of expertise with Microsoft Office, various client record management systems, real-time chat, and web-based technologies.
- Should you be qualified, you must actively pursue individual AIRS certification.
- The drive to look for learning opportunities to foster development on all fronts—personal, professional, and growth.
- Spanish bilingualism is preferred.
