Position Description:

Responsibilities:

  • Deliver exceptional, highly accessible, and patient-centered virtual care services with an emphasis on more urgent or time-sensitive care needs, occasionally providing continuity-based care when appropriate for the patient or on their PCP’s behalf.
  • In certain settings and markets, patients may be seen in person by office-based colleagues.
  • Collaborate with primary care providers to implement innovative, high-quality team-based primary care.
  • Employ multidisciplinary teams that permit a holistic approach to health and wellness.
  • Partner with other clinical and operational stakeholders to implement new clinical initiatives that will improve the value and quality of patient outcomes.
  • Collaborate as required with our product team to develop new clinical and patient solutions.

Requirements:

  • Passion for providing exceptional, high-quality, and value-driven urgent and time-sensitive care within our primary care system, as well as a strong desire to advocate for our patients’ care requirements.

  • Required: Assistant physician/nurse practitioner New York licensure and at least three years of experience in a primary care, urgent care, or emergency room setting.
  • Multiple additional licenses are ideal, with preference for the following states: Illinois, New Jersey, and California
  • Excellent to have: Bilingual with proficiency in English and Spanish
  • Capability to work nighttime shifts
  • A portion of holiday coverage is shared by the care team.
  • Enthusiasm for utilizing technology to provide world-class patient care via our telehealth platform
  • Board Accreditation
  • BLS Certification is Recommended
